Eight weeks after the birth of your baby you will be invited to come for a post natal check. This will also be the time when your baby will be due for their 8 week baby check and first set of immunisations.
Heart Disease & Diabetes Monitoring
The Practice Nurse carries out Chronic Disease reviews aimed at patients with Heart disease and Diabetes annually. These are 30mins appointments aimed at discussing disease control and lifestyle measures.
We will write to you to offer you an appointment to monitor your condition, carry out health checks and arrange any appropriate investigations prior to your appointment. We also run regular audits to ensure that the measures we are implementing are effective and therefore if you are asked to participate in any audits, your co-operation will be very much appreciated.
